These are several forms that are required for the graduate degree programs. Please print a copy and follow instructions on the individual forms for submission procedures.

The Plan of Study should be submitted soon after being accepted into a degree program.  You may confer with your advisor and refer to the Long Range Plan of courses to determine which courses you plan to take/have taken.  This Plan may be changed at any time during your studies, but has to be up to date prior to graduating.

The Application for Graduate Degree should be completed as early as possible during the semester in which you plan to graduate.  Deadlines:  For May graduation - by February of the Spring Semester.  For January graduation - by October of the Fall semester.   Note:  You also need to be registered as a non-resident during your final semester.  Contact the Graduate Studies office for assistance - 434-924-3897.
